Physical Health Audit Nurse Location: Hartwell, Northampton Rate: £20.77 per hour Permanent | Full-time | 35 hours per week (rota-based)Please note: We may be unable to support applications where Home Office visa restrictions prevent candidates from meeting the full contractual hours or requirements of the role.

The Oakleaf group are seeking to recruit a Physical Health Audit Nurse is responsible for coordinating, monitoring, auditing, and improving the delivery of physical healthcare across the service. The role ensures that physical health assessments, audits, care plans, and statutory requirements are completed to a high standard, supporting compliance with regulatory standards and promoting positive health outcomes for service users.
The post holder will work collaboratively with multidisciplinary teams, GPs, and external healthcare professionals to ensure effective monitoring, documentation, and implementation of physical healthcare initiatives.
About the service - The Oakleaf Group
This is an exciting opportunity to join an established multi disciplinary team working within the field of acquired brain injury. The Oakleaf Group currently caters for a male client base specialising in neurological rehabilitation across various sites.
This role is based in one of 4 units at our Hilltop House site at Hartwell in Northampton, currently rated as Outstanding by CQC.
Working for this Leading Care Provider you will be instrumental in the provision of care and therefore it is desirable that you have a proven track record working in a similar position within the care sector.
The Role
You will:
Clinical Governance and Physical Health Monitoring
- Maintain and regularly update the Clinical Health Tracker to ensure accurate recording of all physical health interventions.
- Act as the primary point of contact for GP annual physical health reviews and coordinate completion of annual health checks.
- Oversee and monitor DNACPR documentation and associated trackers.
- Maintain medication profiles and support the safe management of medicines.
- Complete and monitor hospital passports to facilitate continuity of care during hospital admissions.
- Provide oversight of routine physical observations and ensure appropriate escalation where required.
- Support the implementation and monitoring of individual physical health action plans.
Auditing and Quality Improvement
- Conduct regular physical health audit oversight including:
- Tissue viability oversight
- Medication chart audits
- PRN protocol audits
- Medical equipment and medical directory audits
- Monitor compliance with clinic audit action plans and ensure identified improvements are implemented.
- Analyse audit findings and provide feedback to managers and clinical teams to support continuous quality improvement.
Clinical Assessments
- Coordinate and support:
- Oral health assessments
- Annual continence assessments
- Health screening programmes
- Vaccination programmes
- Monitor outcomes and ensure referrals are made where appropriate.
Health Promotion
Promote healthy lifestyles through:
- Smoking cessation support
- Health education
- Physical health promotion initiatives
- Support residents in improving physical wellbeing through evidence-based interventions.
Training and Staff Support
- Support staff competency assessments relating to physical healthcare.
- Deliver or coordinate physical health training sessions, including ABI week where applicable.
- Facilitate regular defibrillator (AED) drills to maintain staff competence.
- Provide clinical advice and support to nursing and care staff regarding physical healthcare.
Nutrition and Specialist Liaison
- Liaise with dietetic services (including liaison with Sam Bailey or relevant professionals) to ensure nutritional care plans are implemented and monitored.
- Support compliance with nutrition and hydration plans.
