About the role
Wiltshire Support at Home provides reablement and care services to people returning home following a hospital stay.
As a First Responder, you will be the first point of contact for new customers, supporting safe hospital discharges and helping people regain confidence and independence in their own homes. You will work closely with occupational therapists and colleagues as part of a trusted, supportive team.
This is an active, community‑based role that requires you to work flexibly across early mornings, evenings, weekends, and bank holidays. You will travel across Wiltshire to visit customers and must be able to respond calmly and professionally to changing situations.
